Your Care Manager will ensure your admission (check-in) and transition into the Santé of North Scottsdale Therapy Program is a successful experience. The Care Manager will coordinate your care with your physician, therapists, and clinical associates and plan your "Bridge to Home."

Santé of North Scottsdale Care Managers place you and your family at the center of healthcare and hospitality service. You are provided a designated Care Manager so you have peace of mind knowing there is a specific professional who is coordinating information about your health status and explaining why particular therapeutic services are important. Care Managers work closely with other health care team members to ensure information is shared in a timely manner. State-of-the-art electronic health records promote greater efficiencies enabling our Care Managers to devote more time to keeping your healthcare team informed, anticipating services and planning for a successful and safe transition home.

Twenty-four hour skilled nursing care is provided by professional, licensed nurses and rehabilitation guest associates (RGA). Our nurses and RGAs help you become knowledgeable so you feel comfortable directing your rehabilitation care. The rehabilitation guest associates are available for your assistance.
